4 | Hello Ellen, We are very excited to host your chaperones and students for your upcoming school trip to the Ashokan Center! This is your weekly upcoming trip reminder.
If your number of student participants or chaperones has changed, please inform us so we can change your numbers. The school gets charged according to our pre-set numbers, not who actually shows up. We want to be sure you are not paying for any extra students or chaperones that are not attending. Medical forms are only required when medications, including OTC's (advil, tylenol, etc.) will, or are anticipated, to be administered. It is very important that we have this information to best support your students health needs. Please be advised, that it is up to medical contact from your school to mark all student submissions "reviewed" to assure that what has been submitted looks accurate. Any uploaded med forms need to be signed by both the parent/guardian and physician as well as stamped by the doctor's office.
